10 weeks ago, I came down with what I thought was just sciatica. When it didn’t go away in about three weeks I went to the orthopedic surgeon. He provided the “Classic“ treatment plan:

1. Medications
2. Physical Therapy
3. MRI
4. If necessary, surgery

1. He prescribed meloxicam for the pain, and gabapentin to reduce the sensitivity of the nerves. Neither one did any good other than just plain old Aleve.
2. With each physical therapy visit, my pain got worse. After the sixth visit, he scheduled an MRI.
3. MRI found that I had arthritis in my spine, spinal stenosis, and bulging discs, with the L5 – S1 ruptured.
4. Orthopedic surgeon wanted to do immediate surgery on L5 – S1, with three days of hospitalization, and a 90 day recovery wearing $1000 back brace 24/7. Then another three day hospital visit for spinal fusion, with 90 days in the back brace again.

Susan did some research and found the Bonati Spine institute in Hudson Florida, about three hours north of us from Fort Myers. They have been providing alternates to orthopedic surgery for 30 years, with a 99.5% success rate. They do it on an outpatient basis with a local anesthetic and “conscious anesthesia”. We chose this option.

I was awake the entire time for each of the four procedures - 1 per disc. They began at the top: my L2 – L3 disc, then L3 – L4, then L4–L5, and then the worst one, L5 – S1. One procedure per day. When they finished the fourth procedure I had no nerve pain. I still have full numbness from my left knee down to the bottom of my foot, which they say will go away in about a week.

It’s expensive, $25,000 per procedure, and it’s out of network on my insurance, so we’ll see if they’ll pay anything. it really doesn’t matter, as I have relief from the pain.

As soon as I get my nerves back below my left knee I’ll be able to walk without a cane.

They have a great website you can read through – it provides a lot of information. You can send them a recent MRI, and they will do a full consultation with you over the phone for free.

So far so good for me and I’ve avoided two painful surgeries with the subsequent 180 days total recovery.

That's great news and happy for you! I'm going to pass your experience on to a family member who had to have emergency spinal surgery after getting thrown off a mechanical bull during a trip to Nashville. They are telling him he may need another surgery.
 
I herniated 2 disks lifting a support beam …… physical therapy almost cured me 20 years ago…. Trust me I know all too well the pain.
 
Wow I feel your pain. I previously had L5-S1 fused and also C5/6 to C6/7 back in 2000 & 2001 due to a car accident. Fast forward to 2019 I had to have C7/T1 fused because apparently when you fuse a disc you put stress on the adjoining discs. Well this past October the Mrs. & I went hiking and clumsy me slipped and fell forward landing on my hands then rolling onto my right shoulder. I got up thinking I was ok. I ended up in the hospital Thanksgiving week due to excruciating sciatic pain so bad I couldn't walk for five days! Long story short I had back surgery on December 27th to relieve the pressure on the nerve. The Doctor did a laminectomy with a microdiscectomy L3/L4 and now I'm on the mend. I'm glad your surgery turned out so well. I'm also walking with a cane and hoping to be done with it once I'm allowed to start PT in another month.

I know your pain. March 8th I am going in for my third back surgery. Going to have the L3 L4 L5 fused. Already have a fusion just above this. I have been in pain since late summer and can't wait to have this done.
 
Been fighting sciatica for years. Been able to control it with stretching exercises. Hoping to avoid surgeries. Glad you found relief without fusion. Thanks for sharing alternatives.

Interesting to hear of alternative procedures.
In the fall of 2019, I had the L5/S1 fusion done for a 30% slip. It was 6 mos for back to work and 12 for full recovery. No brace. My trade is pretty physically demanding though and may be the difference from the numbers you posted. The back feels good and strong still, but the sciatica crap is slowly starting to show up again.
Would like to hear how it all works out down the road and hope you have a quick and successful recovery.

Glad to hear about that. I had a stint with prostate cancer a few years ago, and after hearing the plan of attack, I researched my own. Dr. Danny Sperling, West Palm Beach (and other places), used hi-def MRI guided Focal Laser Ablation to do the deal, utilizing an available existing opening in my body. I had to buy him a car, and BCBS said they never heard of the procedure back then, so it was off the table for coverage. Best car I've ever bought. Good luck with the forward progress.
 
I had really bad sciatica a few years back. Apparently I had been walking around for about a year with a cracked vertebrae. Their play book is always incremental, with surgery being the last resort. Did the injections, the physical therapy. Heck I even tried acupuncture. Spinal Fusion was the fix, and I wish I had insisted on going that route to begin with. Would have saved a whole lot of pain and expense.
 
About 10 years ago I had unbearable sciatica. I told my doc I needed a referral to a surgeon, I had had enough. He talked me into seeing a chiropractor he used. I was not a believer, but I liked my doc so I went. The chiro put me through a couple exercises. He tells me I have the shortest hip flexors he's ever seen. My hip flexors were so tight they were compressing my lower back and causing bulging discs. Two weeks of stretching twice a day and 5 office visits later I was pain free. It was crazy. Glad I listened to my doc.
