wide load permit Ohio - cost ?

I am looking at 9' beam boats. I am having a hard time finding info on the price of an annual wide load permit for this size in Ohio...anybody know offhand ?

are you talking one time or multiple use? Her is the link to the 2003 fee scheudle. I think these are intended for one time movement though.
